Prompts to check yourself
  • Is your ask specific and time-bound — not just "please review"?
  • Did you say you have consent to refer and share?
  • Did you close with a read-back so the receiver accepts the action?
  • Are your observations relevant to this referral only?
List only what the receiver needs to know to act safely.
Compare with a model answer
Minimum necessary, e.g.: patient name/identifier; the specific clinical need (high MUST score, weight loss); the precise request and urgency; relevant history only; and confirmation that consent was obtained. Leave out unrelated diagnoses, social detail with no bearing on the referral, and anything the receiver doesn't need to act.
