Know the surfaces and aids — assess the risk, offload the pressure, and never drag or rub.

A realistic equipment explorer and pressure-injury-prevention simulator: identify the support surfaces, cushions and repositioning aids — then assess risk, inspect the skin, apply the SSKIN bundle, reposition safely on a 30° tilt with a slide sheet, offload heels, and recognise and escalate a pressure injury.

Three "never" rules: never use a ring/donut cushion (it concentrates pressure around the rim and reduces perfusion); never massage or rub bony prominences or reddened skin (it causes further tissue damage); never drag a person — use a slide sheet/hoist (shear and friction injure skin and are a manual-handling risk). Risk assessment, skin inspection, the prevention bundle, aids per the care plan, repositioning and escalation are within EN scope, in consultation with the RN.
